After School Forms
PS 154 After School runs three enrichment club sessions per year: Fall, Winter, and Spring. Please use your Amilia SmartRec account to register for clubs during our open registration period. Fall 2024 registration period is from 8/5 - 8/19. Starting 8/20/24 if you would like to drop a club for your child, please use this form.
Please request only one club drop per form. Feel free to submit multiple forms if needed.
No requests are complete until you receive a confirmation email from the PS 154 After School Staff.

Extended Day services are billed monthly. If you need to increase, decrease, or change the days your child joins us for Extended Day, you may do so at the beginning of any month as long as there is space available to accommodate your needs.
Please use this form to request any change to your child's extended day schedule.
Please request changes for only one child per form.

Please fill out this form if you would like to give permission for your 3rd, 4th, or 5th grader to self-dismiss from extended day and/or enrichment clubs.
This is a form to verify financial assistance for the PTA of PS 154 After School Program. Please be advised that full scholarships are prioritized for families who are currently receiving government subsidies such as SNAP, Medicaid, Rent Assistance, and families who are asylum seekers.
PS 154 After School offers a drop-in option for grades K-5 based on availability.
